What To Expect On Your Information Call

You'll meet by phone with the Program Specialist. The objective of the conversation is to ensure that you qualify for the program and that you fully understand the program and feel that it would be beneficial for you. The conversation is loosely divided into three parts:

​

1. You will share your heath journey to this point, your experience with the healthcare system related to your condition, other health solutions or lifestyle changes you may have tried already, and your perceptions of how effective medications have been for you over time.

 

2. Your Program specialist will provide you a description of the program, your care team, and what to expect. You are invited to ask as many questions as you wish to ensure your interest in participating in the program and the study.

 

3. Assuming that you wish to take part in the program, you and your program specialist will arrange a time for you to meet your physician and health coach.
